Church Benevolence Partner Commitment rev 02.pdf A church uses this form to partner with W.D.C.

Once the commitment form is processed, we will send the church a form to complete to refer a client to us for a vehicle repair.

If a church has a widow or a single-parent who needs a vehicle, they can apply using this form. This is only for church members from partner churches who have exhausted all other options.
